These are recommendations, not fixed rates. *Independent providers determine their own prices based on their area, services, and operating costs.

The form below helps us understand whether you're interested in providing services and which areas you may be able to serve as the network grows. Expressing interest doesn't obligate you to participate or guarantee a service territory.
